[FGCMH Core] CC and Concensus Handbooks

Hi all

The document review committee is still attempting to resolve concerns
with the documents.

I consider the two attached files to be the current "working copies"
of the documents.  They are the same documents submitted previously by
Mark with my edits shown in blue.  They have not been approved yet by
the other members of the committee and should not be regarded as being
endorsed by anyone other than me.  I am submitting them now in case we
have time to go over them as a group at the CCC meeting and hash out
any remaining concerns.
